This form captures student information only to support program planning and follow-up through the school. The school will obtain parental/guardian permission.

What we collect (student info only)

We keep this simple and useful — enough to support the student experience and program logistics.

  • Student basics (name, grade, age range)
  • Golf experience level (new / some experience)
  • Interests and goals (confidence, teamwork, leadership)
  • Scheduling needs and general notes (as shared by school)
  • Any accommodations the program team should consider

This form is not a waiver. Parental/guardian permission and participation approvals are managed by the school.
